Connect, Create, Contribute

May is Older Americans Month. In honor of this, the Administration for Community Living has made their theme 2019 Connect, Create, Contribute, encouraging older adults to live active, thriving lives, and asking loved ones to participate in their success. Alexian PACE would like to share with you how we strive to bring this vision to life for all the adults we serve, not just this month, but all throughout the year!


Connect


One of the primary functions of our program is to care for the hearts of older adults living at home by connecting them with community. We do this in a number of ways, beginning with our staff: members of PACE are brought into a compassionate family of care, and have access to a team which includes medical professionals, physical and occupational therapists, dietitians, social workers, and more.

Some of our other resources for connection include:


Day Centers: PACE has three day centers for our participants to enjoy. Here our staff host special events, games, activities and provide healthy meals. These are fun opportunities for our participants to connect with their community and form new friendships.


Transportation: Transportation is a key piece of what PACE has to offer. Drivers not only provide transport to and from medical appointments and our day centers, but they develop a personal relationship with the adults they serve.


Home healthcare: Our caregivers and nurses that make home visits do far more than offering medical assistance and help around the house: they invest in the lives of the families they serve, and become part of their community.



Create


Our staff at Alexian PACE also cares deeply about engaging the minds of our participants, to promote quality of life and personal enrichment. We take a holistic approach which includes:


Memory care: We provide a memory care unit on our campus which offers specialized nursing and personal care for individuals who need assistance in this capacity. Memory care has been shown to improve quality of life in many ways, from reducing fall risks to maintaining vital and loving relationships.  Memory care also promotes peaceful and meaningful engagement for those who need a calming environment and compassionate caregivers.


Events and opportunities to create: From gardening, live music, Tai Chi, crafting and special outings, we love to provide an atmosphere that fosters creation, inspiration and ongoing education. Engaging the mind’s creativity helps to form new neural pathways, which is effective for promoting brain health, memory and mental agility. Our desire is to enhance the lives of our participants in all directions by providing opportunities to grow and engage their passions.


Contribute


The team at PACE believes that our participants are full of valuable life experiences, talents and wisdom. By caring for their physical health, we hope to support them in using these gifts to benefit and enrich their families and communities. Some of our physical care programs include:


Healthcare: At our day centers, participants can build strength and maintain health through the care of our occupational and physical therapists, dieticians who support their nutritional needs, and other medical professionals who monitor their care. Our rehab team partners with PACE participants to promote safety by preventing falls and maximizing physical function.  We also coordinate all specialist appointments as a part of the comprehensive PACE model of care.


Personal care: Our focus on caring for the physical needs of our participants is not limited to medical concerns. A beauty shop is available at our center, and home care aides help folks with bathing, dressing and feeling outwardly prepared for their day.


Onsite pharmacy: Staying on top of medications is incredibly important for long and short-term health. We provide and deliver all prescriptions and over the counter medications that are needed.
